FYI: the pinky gesture is used to indicate a girlfriend or (female) lover.
It’s usually used when you don’t want to outright ask or are implying.
For example, your guy friend is talking to a girl and seems really into her, so you might turn to your other friend and be like “is she his…(pinky up)?” to ask if that’s his girlfriend.
He’s being a smidge insulting by saying she’s thought of as “just” the Duke’s lover, but I don’t believe it’s an inherently derogatory gesture, (unless it’s used in a gay couple to imply one guy is a bottom/submissive.)
